MVR — To obtain a copy of your 7-year driving record (MVR), visit your local DMV in person or request one online to have a non-certified copy emailed to you via colorado.gov/dmv. The cost is approximately $9.00. Send a copy to jobs@caringhandstransport.net or fax to 720-485-3752.
Drug Testing — PreCise Monitoring, 10370 Colorado Blvd, Thornton, CO 80229, 303-920-5959. Hours: Monday – Friday 8:00 AM–8:00 PM, Saturday 9:00 AM–1:00 PM. A 10-panel rapid screen drug test is required (tests for amphetamine, benzodiazepines, cocaine, opiates, marijuana, barbiturates, phencyclidine, alcohol, methadone, and propoxyphene). Cost: $30.00.
Fingerprinting — Pre-enroll with IdentoGO at uenroll.identogo.com or call (844) 539-5539. Use Service Code 25YQ2S and select PreCise Monitoring (10370 Colorado Blvd, Thornton, CO 80229) as your location to complete fingerprinting and drug testing the same day. CBI/FBI fingerprinting fee: $39.50, plus a $10.00 processing fee (total $49.50).
DOT Physical — Driver's Choice, Pine Creek Center, 4755 Paris Street, Suite 150, Denver, CO 80239, 720-538-2726. Walk-ins welcome Monday–Friday 8am–5pm, or book at dot-physical-exam.com. Cost: $60.00.

Please bring a copy of your photo ID to each appointment. A staff member will assist you with fingerprinting and drug testing steps 2 & 3. Reimbursement is provided after 90 days of employment.
